How do I schedule an appointment?

What should I bring to my first appointment?
Your ID, insurance cards, a physician signed referral (if you have one), and your new patient registration packet. You can print & fill out this packet before your first appointment by downloading it here.
Does insurance cover my treatment?
Most insurance plans have some type of physical therapy/occupational therapy coverage. We encourage patients to check their individual plans. Before scheduling, we will confirm insurance benefits and prior authorization information.
Do I need a referral from my doctor?
You do not need a referral to schedule an initial evaluation, but we will need your physician’s information to send the plan of care for a signature.
What type of payment do you accept?
Mastercard, Visa, American Express, Discover, cash, or check.
How long will my therapy sessions be?
We allow up to one hour per treatment session.
