Important Information about the Coronavirus COVID-19
Coronvirus (COVID-19) Update: We are following the current CDC guidelines and Governor Holcomb’s statewide address on Monday, March 23, 2020 that is in effect from March 25 to April 7, 2020 and could be extended if the outbreak warrants it.
Currently, it appears that Physical and Occupational Therapy in Indiana has been designated as an essential business/activity and was backed by the INAPTA.
Rehab for Life will continue to follow the current CDC guidelines and all state mandates to assure the protection and safety of our patients and staff. If you have fever or respiratory symptoms such as shortness of breath or cough, or have either traveled internationally in the past two weeks or have had contact with someone who is suspected to have Coronavirus, please call your primary healthcare provider before scheduling or attending an appointment with us.
We reserve the right to deny treatment to any individual if we feel that they, the staff, or other patients may be at increased risk.
Rehab for Life Screening Checklist
1. Do you have a fever (greater than 100 degrees) or signs/symptoms of lower respiratory illness (e.g. sore throat, cough, new shortness of breath, fever)?
2. Have you traveled outside the United States within the last 14 days?
3. Have you had contact with someone who is suspected to have Coronavirus or is ill with fever, cough, or new symptoms of shortness of breath?
4. Have you been in contact with or worked in another health care setting that has confirmed COVID-19 cases? (May change as COVID spreads in the community)
If the individual being questioned answers yes to any of the questions, please ask them to immediately leave the building
If the individual being questioned answers no to any of the questions, please take temperature
